Terms of Service

Effective Date: August 18, 2026 Last Updated: August 18, 2026

Innate Wellness DFW LLC

These Terms of Service ("Terms") govern your access to and use of the website innate-dfw.com (the "Site") and the services described on it. The Site is operated by Innate Wellness DFW LLC, a Texas limited liability company, referred to in these Terms as "Innate Wellness," "the Company," "we," "us," or "our." By accessing or using the Site, or by booking a consultation or enrolling in membership, you agree to be bound by these Terms. If you do not agree, do not use the Site or our services.

1. Eligibility

You must be at least 18 years of age to enroll, to book services for yourself, or to enter into any agreement with us. Services for minors, where offered, require the consent and presence of a parent or legal guardian. By using the Site, you represent that you meet this requirement and that any information you provide is accurate and complete.

2. Use of the Site

You may use the Site for lawful, personal, non-commercial purposes only. You agree not to:

  • Use the Site in any way that violates applicable law or regulation

  • Misrepresent your identity, age, or medical information when contacting us or submitting a form

  • Attempt to gain unauthorized access to any portion of the Site or its systems

  • Interfere with or disrupt the operation of the Site

  • Use automated means to access or scrape the Site without our written permission

3. No Medical Advice; No Provider-Patient Relationship

The content on this Site is provided for general informational and educational purposes only and does not constitute medical advice. Viewing this Site, contacting us through it, or submitting a form does not create a provider-patient relationship. A provider-patient relationship is established only after you have formally enrolled, completed required intake and consent documentation, including a good faith examination where required, and been accepted as a patient.

Our Medical Disclaimer is incorporated into and forms part of these Terms. In the event of a conflict between these Terms and the Medical Disclaimer on a clinical matter, the Medical Disclaimer controls.

4. Our Services

Innate Wellness provides concierge primary care and health optimization services, including functional and integrative medicine, hormone optimization, peptide therapy, medical weight management, longevity and performance testing, corporate and executive health programs, and chiropractic services. Not all services are available to every patient. All services are subject to clinical evaluation, medical appropriateness, and acceptance.

Nothing on this Site constitutes an offer of guaranteed treatment, outcome, or result.

Tailored by Innate LLC

Intravenous nutrient therapy and related injectable wellness services are offered by Tailored by Innate LLC, a separate legal entity affiliated with us through common ownership. Those services are provided under that entity’s own terms of service, privacy policy, and consent documentation, and are booked through its own website. These Terms do not govern services provided by that entity. Members may be eligible for preferred pricing as separately described.

Chiropractic services

Chiropractic care is provided through the practice by a Doctor of Chiropractic licensed by the Texas Board of Chiropractic Examiners, practicing within the chiropractic scope of practice as defined by Texas law. The treating Doctor of Chiropractic exercises independent clinical judgment within that scope and is not supervised by, and does not practice under delegation from, any physician or physician assistant. Chiropractic services are billed separately on a per-visit basis and are not included in any membership. A current fee schedule is available on request.

5. Membership, Fees, and Payment

Innate Wellness operates on a cash-pay model. Membership is offered in tiers, each with a defined scope of clinical care, provider access, and included testing. Membership covers the clinical relationship, including consultations, provider access, protocol design, and ongoing management. Medications, peptides, injectables, intravenous therapy, chiropractic care, and certain testing are billed separately, at member pricing where applicable.

Membership terms, fees, billing, renewal, cancellation, and refund policy are governed by the separate membership agreement you enter into upon enrollment. Current rates and inclusions are provided in writing at enrollment. Prices are subject to change. In the event of a conflict between these Terms and your membership agreement, the membership agreement controls with respect to the subject matter it addresses.

Fees for services not covered by membership are payable at the time of service.

6. Insurance and Laboratory Billing

We do not bill insurance, Medicare, or Medicaid for membership fees, clinical services, or any service we provide. Where laboratory testing is ordered, the laboratory may bill your insurance directly under its own arrangement with your plan. Any such billing is between you and the laboratory. We are not a party to it, we do not receive payment from your insurer, and we cannot guarantee coverage or the amount you may be charged.

7. Eligibility for Treatment, Intake, and Prescribing

Certain protocols, including hormone therapy, peptide therapy, and medical weight management medications, require a comprehensive baseline evaluation before any prescribing decision. We will not prescribe without appropriate clinical justification.

We reserve the right, in the exercise of clinical judgment, to decline to initiate, to modify, or to discontinue any protocol we determine is not clinically appropriate for you, and to decline or discontinue the provider-patient relationship consistent with applicable professional obligations. You are responsible for providing complete and accurate health information at intake and for promptly reporting any change in your health status, medications, or pregnancy status.

8. Appointments, Cancellation, and Communication

Please provide as much notice as possible if you need to cancel or reschedule an appointment. Late cancellations and repeated missed appointments without notice may be subject to a fee. Where a fee applies, it is disclosed in your membership agreement or applicable service agreement.

Non-urgent communication with your clinician is handled through the channels specified in your membership tier and through the secure messaging platform we designate. Communication channels are not monitored continuously and should not be used for urgent or emergent concerns. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

9. Assumption of Risk

You acknowledge that medical and wellness services, including hormone and peptide therapy, medical weight management, and chiropractic care, carry inherent risks. Those risks are described in the consent documentation you review and sign before treatment. By receiving services, you voluntarily assume those risks.

This provision does not waive any liability that cannot be waived under Texas law, and does not limit your rights with respect to the actual provision of medical care.

10. Termination

You may cancel your membership or end the relationship in accordance with the terms of your membership agreement. We may suspend or terminate your access to the Site, your membership, or the provider-patient relationship where you materially breach these Terms or your membership agreement, where payment is not made when due, where you provide materially inaccurate health information, or where continuing the relationship is not clinically appropriate.

Termination of the clinical relationship is handled consistent with applicable professional obligations, including reasonable notice and assistance in transferring care where required. Amounts owed for services already rendered remain payable. Treatment of prepaid or unused amounts is governed by your membership agreement.

11. Third-Party Links and Clinical Partners

The Site may reference or link to third parties, including clinical partners such as diagnostic, laboratory, and imaging providers. Full-body imaging referenced on the Site is provided by LifeGenix Institute, an independent business. Referrals to any partner are made at your request or with your consent. We do not control and are not responsible for the content, services, pricing, privacy practices, or regulatory compliance of any third party, and services provided by them are governed by their own terms and policies rather than these Terms. References to partners do not constitute endorsement of, or responsibility for, services rendered by those partners.

12. Intellectual Property

All content on the Site, including text, graphics, logos, the Innate Wellness names and marks, images, and design, is owned by or licensed to Innate Wellness and is protected by intellectual property laws. You may not reproduce, distribute, or create derivative works without our prior written consent.

13. Electronic Communications, Text Messaging, and Signatures

By using the Site or providing your email address or mobile number, you consent to receive communications from us electronically. You agree that electronic signatures, agreements, notices, and disclosures satisfy any legal requirement that such communications be in writing.

Where you provide a mobile number and affirmatively opt in, you consent to receive text messages from us relating to appointments, care coordination, billing, and, where separately authorized, practice updates. Message frequency varies. Message and data rates may apply. You may opt out at any time by replying STOP, and may request help by replying HELP. Consent to receive text messages is not a condition of receiving care. Opting out of clinical or appointment messaging may limit our ability to provide certain services.

You may withdraw consent to electronic communications generally by contacting us, though doing so may limit our ability to provide certain services.

14. Disclaimers

THE SITE AND ITS CONTENT ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED, SECURE, OR ERROR-FREE.

15. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY TEXAS LAW, INNATE WELLNESS AND ITS OWNERS, PROVIDERS, CONTRACTORS, AND STAFF SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF, OR INABILITY TO USE, THE SITE OR ITS CONTENT. THIS LIMITATION DOES NOT APPLY TO LIABILITY THAT CANNOT BE LIMITED OR EXCLUDED UNDER APPLICABLE LAW, INCLUDING LIABILITY FOR GROSS NEGLIGENCE OR INTENTIONAL MISCONDUCT, AND INCLUDING CLAIMS ARISING FROM THE ACTUAL PROVISION OF MEDICAL CARE, WHICH ARE GOVERNED SEPARATELY.

16. Indemnification

You agree to indemnify and hold harmless Innate Wellness from any claims, damages, or expenses arising out of your misuse of the Site, your violation of these Terms, or your provision of inaccurate health information.

17. Governing Law and Venue

These Terms are governed by the laws of the State of Texas without regard to its conflict-of-law principles. Any dispute arising under these Terms shall be brought exclusively in the state or federal courts located in Dallas County, Texas, and you consent to the jurisdiction of those courts, unless otherwise required by applicable law.

18. General Provisions

Severability

If any provision of these Terms is held invalid or unenforceable, that provision will be modified to the minimum extent necessary to make it enforceable, and the remaining provisions will continue in full force and effect.

Entire agreement

These Terms, together with our Privacy Policy, Notice of Privacy Practices, and Medical Disclaimer, constitute the entire agreement between you and Innate Wellness DFW LLC regarding your use of the Site, and supersede any prior understanding on that subject. Where you have entered into a separate written agreement with us, including a membership agreement, a service agreement, or a consent to treatment, that agreement controls with respect to the matters it addresses.

Assignment

You may not assign or transfer any rights under these Terms. We may assign these Terms to any affiliate, successor entity, or entity formed in connection with a reorganization of our corporate structure, including any professional entity or management entity formed as part of a restructuring.

Waiver

Our failure to enforce any provision of these Terms is not a waiver of that provision or of any other provision, and does not affect our right to enforce it later.

19. Changes to These Terms

We may modify these Terms at any time. Changes are effective when posted with an updated "Last Updated" date. Your continued use of the Site or our services constitutes acceptance of the revised Terms.
