Medical Disclaimer
Innate Wellness DFW LLC
This Site is operated by Innate Wellness DFW LLC, a Texas limited liability company, referred to in this Disclaimer as "the Company," "we," "us," or "our." References to "Innate Wellness" or "the practice" mean the Company and the licensed clinical personnel providing services through it.
1. Informational Purpose Only
The information provided on innate-dfw.com (the "Site"), including all text, graphics, descriptions of services, protocols, and other materials, is for general informational and educational purposes only. It is not intended to be, and should not be interpreted as, medical advice, diagnosis, or treatment. The content is not a substitute for professional medical judgment, examination, or care, and no content on this Site should be used to self-diagnose, self-treat, or decide whether any particular therapy is appropriate for you.
2. No Provider-Patient Relationship
Accessing this Site, reading its content, submitting an inquiry form, or otherwise contacting Innate Wellness through it does not establish a provider-patient relationship. Such a relationship is created only after you have completed a formal evaluation, including a good faith examination where required, executed all necessary consent and intake documentation, and been formally accepted as a patient of the practice.
3. Not a Substitute for Emergency Care
Innate Wellness provides concierge primary care and health optimization services on a scheduled, non-emergency basis. Our services are not a substitute for emergency medical care, urgent care, or hospital services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this Site, our messaging platform, or any other contact method offered by the practice to report an emergency.
4. Always Consult a Qualified Provider
Never disregard, avoid, or delay obtaining professional medical advice because of something you have read on this Site. Always seek the advice of a qualified healthcare provider with any questions you may have regarding a medical condition, symptom, medication, or health objective.
5. Clinical Oversight and Provider Credentials
Medical services at Innate Wellness are provided by a licensed Physician Assistant (PA-C) practicing in accordance with Texas law and applicable delegation and supervision requirements, under physician medical oversight.
Chiropractic services are provided through the practice by a licensed Doctor of Chiropractic exercising independent clinical judgment within the chiropractic scope of practice. A Doctor of Chiropractic is not a physician and does not practice under physician supervision or delegation.
References on this Site to "clinician," "provider," or "your dedicated clinician" refer to licensed clinical personnel practicing within their respective scopes of practice. Nothing on the Site should be read to represent that services are provided by a physician unless expressly stated.
6. Clinician-Directed, Individualized Protocols
All protocols offered by the practice, including hormone optimization, peptide therapy, medical weight management, and longevity and performance protocols, are designed and overseen by your clinician based on a comprehensive baseline evaluation and are adjusted through ongoing monitoring. No protocol is offered without appropriate clinical assessment first. Descriptions of these services on the Site are educational and do not represent a recommendation that any particular therapy is suitable for you. Suitability is determined solely through individual clinical evaluation.
7. Individual Results Vary
Innate Wellness provides personalized, individualized care. Any descriptions of services, protocols, or potential benefits on this Site are general in nature and reflect typical wellness objectives rather than promised outcomes. Results depend on individual factors including biology, adherence, and medical history. We make no guarantee, representation, or warranty regarding any specific result, outcome, or benefit.
Statements on this Site have not been evaluated by the U.S. Food and Drug Administration unless expressly stated, and our services are not intended to diagnose, treat, cure, or prevent any disease except as delivered through individualized clinical care.
8. Hormone Optimization (TRT and BHRT)
Testosterone replacement therapy and bioidentical hormone replacement therapy carry known risks and require ongoing laboratory monitoring, including markers such as hematocrit, PSA, and estradiol. These protocols are not appropriate for everyone and will be initiated only after a comprehensive hormone panel and clinical evaluation. Risks, benefits, alternatives, and monitoring requirements are reviewed with you and documented in the informed consent you sign before therapy begins.
9. Peptide Therapy and Compounded Medications
Certain medications used in our protocols, including certain peptides, are compounded and are not independently evaluated or approved by the U.S. Food and Drug Administration for the specific indication prescribed. Compounded medications are prepared by licensed pharmacies operating under applicable state and federal regulations, including the compounding provisions of the Federal Food, Drug, and Cosmetic Act.
Because compounded preparations are not FDA-approved products, their safety, efficacy, and manufacturing are not subject to the same premarket review as approved drugs. Your clinician will discuss the available evidence, the known risks, and the alternatives for any compounded medication before it is included in your protocol.
10. Medical Weight Management
Weight management protocols, including any prescription medications used, are individualized based on your health history and are monitored on an ongoing basis. Certain medications used in weight management may be prescribed for indications, or in formulations, that differ from their FDA-approved labeling. Where that is the case, your clinician will discuss it with you before prescribing. Results vary by individual and are not guaranteed.
11. Longevity, Performance, and Imaging Testing
Biomarker panels, VO2 max testing, biological age analysis, and full-body imaging offered through the practice or through our clinical partner LifeGenix Institute are wellness and screening tools. They are not a substitute for diagnostic evaluation of a specific medical concern, and they are not intended to rule out the presence of disease.
Screening tests of this kind can produce findings of uncertain significance, incidental findings unrelated to your health goals, and both false positive and false negative results, any of which may lead to additional testing. Findings that warrant further workup will be referred to an appropriate provider or specialist. You should continue to receive age-appropriate and risk-appropriate preventive screening from a qualified provider regardless of any testing performed through this practice.
12. Chiropractic Services
Chiropractic care described on this Site is provided through the practice by a Doctor of Chiropractic licensed by the Texas Board of Chiropractic Examiners, practicing within the chiropractic scope of practice as defined by Texas law. Chiropractic care is a distinct discipline and is not the practice of medicine.
Chiropractic treatment, including spinal manipulation, carries inherent risks. Those risks, including the rare but serious risks associated with manipulation of the cervical spine, are described in the chiropractic informed consent you review and sign before treatment. Descriptions of chiropractic services on this Site are educational and do not represent a recommendation that chiropractic care is suitable for you. Suitability is determined solely through individual evaluation by the treating Doctor of Chiropractic.
No claim is made that chiropractic care cures or lessens any condition outside the chiropractic scope of practice, and no specific result is guaranteed.
13. Your Health History and Contraindications
Certain health conditions, medications, supplements, allergies, and pregnancy or the possibility of pregnancy may make specific protocols inappropriate, unsafe, or in need of modification. You are responsible for disclosing your full and accurate medical history during intake and for notifying the practice promptly of any change in your health status, medications, or pregnancy status while under our care.
Clinical decisions are made based on the information you provide. Incomplete or inaccurate disclosure limits our ability to determine whether a protocol is appropriate and safe for you.
14. Third-Party Services and Clinical Partners
This Site references clinical partners who provide certain services, including but not limited to diagnostic imaging and laboratory testing, at their own facilities and under their own clinical responsibility. Full-body imaging referenced on this Site is performed by LifeGenix Institute. Innate Wellness does not perform those services and is not responsible for care rendered by third parties.
Each such partner is an independent entity. Its services, reports, records, privacy practices, regulatory compliance, and terms are governed by its own clinical, legal, and regulatory standards, not those of Innate Wellness. Any service rendered by a partner is subject to that evaluating provider’s independent clinical judgment.
15. External Information and References
The Site may reference general scientific or health information, published research, or third-party resources. Such information reflects general understanding at the time of writing, may not reflect the most current research, and should not be relied upon as a basis for any health decision without consulting a qualified provider. Links to third-party websites are provided for convenience only and do not constitute an endorsement of their content.
16. Changes to This Disclaimer
We may update this Medical Disclaimer from time to time to reflect changes in our services, applicable law, or clinical practice. The Effective Date and Last Updated date at the top of this page indicate when it was last revised. Your continued use of the Site after any revision constitutes acceptance of the revised Disclaimer.
