Innate Wellness DFW LLC
Innate Wellness DFW LLC, a Texas limited liability company ("Innate Wellness," "the Company," "we," "us," or "our"), respects your privacy and is committed to protecting the personal information you share with us. This Privacy Policy explains what information we collect through our website innate-dfw.com (the "Site"), how we use and safeguard it, and the choices available to you. By using the Site, you agree to the practices described in this Policy.
1. Scope of This Policy
This Policy applies to information collected through our Site. It does not govern protected health information created or received in the course of the provider-patient relationship, which is separately governed by our Notice of Privacy Practices provided under the Health Insurance Portability and Accountability Act and applicable Texas law, including the Texas Medical Records Privacy Act.
Information you submit through website forms is treated as confidential but does not by itself create a provider-patient relationship or constitute a medical record until you have formally enrolled and completed intake.
Tailored by Innate LLC is a separate legal entity affiliated with us through common ownership. Information collected through the Tailored by Innate website is governed by that entity’s own privacy policy and Notice of Privacy Practices, not by this Policy.
2. Information We Collect
Information you provide directly
Name, email address, telephone number, the service or membership tier you are inquiring about, and any details you include when you complete a contact or inquiry form, request a consultation, or communicate with us.
Health-related information you volunteer
If you choose to describe your health goals, symptoms, or interests in a form or message. You are not required to submit sensitive health information through the Site. Please do not include detailed medical history, diagnoses, medication names, or dosages in a website contact form. That information is collected securely during your clinical intake and is protected as described in our Notice of Privacy Practices rather than under this Policy.
Payment information
Processed through our third-party payment processor. We do not store full payment card numbers on our systems.
Automatically collected information
IP address, browser type, device information, pages visited, and referring URLs, collected through cookies and similar technologies to operate and improve the Site.
